Comprehensive Evaluation Fee: $4,650
Includes intake, testing, scoring, interpretation, written report, feedback session, and support for implementation of recommendations.
If families choose not to proceed after the intake appointment, only the intake fee ($350) is charged.
My goal is to provide meaningful clarity, targeted recommendations, and a comprehensive understanding of your child’s strengths and challenges.
Dr. Hampton is an out-of-network provider and does not bill insurance directly.
Following completion of the evaluation, families receive a detailed superbill that can be submitted to insurance for possible reimbursement.
We recommend you check with your insurance plan about out-of-network benefits prior to your first appointment. Common billing codes include 90791, 96132, 96133, 96136, 96137, 96138, 96139
